Emerging Technologies: Topical NSAIDs for Pain

Course Description
|
|
|
Nonsteroidal anti-inflammatory drugs (NSAIDs) and
cyclooxygenase-2 selective inhibitors (coxibs) are effective
analgesic and anti-inflammatory drugs, and an important part
of the pharmacologic approach to pain relief. However,
nonselective NSAIDs (NS-NSAIDs) are associated with well
recognized upper and lower gastrointestinal (GI) adverse
effects. While coxibs are differentiated by lower reported
rates of GI complications, all these drugs may be associated
with increased risk of cardiovascular harm. Serious GI and
cardiovascular events may be rare, but they are important
because they may not be reversible and can be life-threatening.
The recognition of the magnitude of risk for both NS-NSAIDs
and coxibs has increased focus on the use of nonsystemic
topical NSAIDs to address certain safety concerns and provide
physicians with expanded options to treat pain. Topical
administration of NSAIDs offers enhanced delivery of
therapeutic drug concentrations to the target tissues to relieve
localized pain, but with potentially fewer systemic adverse
effects due to lower systemic absorption, as well as less
potential for drug-drug interactions and absence of the firstpass
hepatic effect.

experience, for the analgesic efficacy of locally applied
topical NSAIDs
• Discuss the relative safety profile, including the risk,
extent, and clinical relevance of potential cutaneous and
systemic adverse reactions of topical NSAIDs
• Differentiate the pharmacokinetic characteristics and
mechanisms of action of topical NSAID compounds
• Identify patients who would most benefit from topical
NSAIDs to determine where these drugs fit in the
armamentarium for pain management
